Your personal data may be processed by Özel CM Park Ağız ve Diş Sağlığı Polikliniği within the scope of the Law on the Protection of Personal Data No. 6698 (“Law”). Under the Law, Özel CM Park Ağız ve Diş Sağlığı Polikliniği is considered as the “Data Controller.”

As the “Data Controller” (Dedebaşı Mh. Ordu Blv. 74/C Karşıyaka, Izmir, Türkiye), Özel CM Park Ağız ve Diş Sağlığı Polikliniği places the utmost importance on the security of your personal data and its lawful processing. With this awareness, we are committed to processing and protecting the personal data of our patients, potential patients, employees, employee candidates, interns, supplier/subcontractor employees and representatives, shareholders, business partners, visitors, and other third parties in compliance with the Law. Acting with this responsibility, we process your personal data within the limits mandated by the Law, as described below.

PURPOSE OF PROCESSING YOUR PERSONAL DATA

Processing of personal data refers to any operation performed on data, whether fully or partially automated, or through non-automated means that form part of a data recording system, such as collection, recording, storage, retention, alteration, reorganization, disclosure, transfer, acquisition, making available, classification, or restriction of use. Your personal data is processed in line with the purposes and legal grounds set forth under the Law and secondary regulations.

Accordingly:

• Your personal data such as name, surname, Turkish ID number, signature, email, address, phone, property purchase details, identity information, and payment information may be processed for the establishment and performance of contracts (e.g., real estate sales promise contracts).
• Your personal data such as name, surname, email, and phone may be processed, with your consent, for the purpose of informing you about our campaigns.
• Your personal data such as identity information and property details may be processed for transactions to be carried out in your name, including land registry, notary, subscription transactions, and preparation of power of attorney for company employees to act on your behalf.
• Your personal data may also be processed for enabling you to benefit from our products and services, customizing them according to your preferences and needs, implementing our human resources policies, ensuring legal and commercial security, and determining and implementing our commercial and business strategies.

TRANSFER OF YOUR PERSONAL DATA

Collected personal data may be transferred for the following purposes: enabling you to benefit from our products and services, customizing them according to your preferences and usage habits, ensuring the legal and commercial security of our company and persons in business relations with our company (e.g., construction, sales, administrative operations, physical security of company premises, complaint management, reputation research, event management, legal compliance, audits, finance), determining and implementing business strategies, and executing our human resources policies.

Within this scope, your data may be transferred to our business partners, affiliates, suppliers, company representatives, shareholders, notaries, banks, project partners, legally authorized public institutions and private parties, and data processors that store personal data. Data may also be shared domestically in accordance with Article 8 of the Law, and abroad if the conditions of Article 9 are met.

In addition, with your consent, we may use your contact details for commercial electronic messages to inform you about products, services, promotions, and special offers. Such communication will be stopped upon your request.

METHOD AND LEGAL BASIS OF DATA COLLECTION

Your personal data including identity, contact, professional, commercial, and financial information may be collected by automated or non-automated means (e.g., forms, printed documents, electronic or verbal communication). Data is processed based on the purposes stated above, within the legal framework, and in line with Articles 5 and 6 of the Law.

RETENTION AND STORAGE PERIOD OF YOUR PERSONAL DATA

If a specific retention period is stipulated in applicable laws or regulations, your personal data will be stored for such period. If no legal period is defined, data will be stored as long as required for the services we provide and in accordance with our commercial practices. Once the processing purpose ends, or retention periods expire, personal data is deleted, destroyed, or anonymized.

If the processing purpose ceases but data is still needed for possible legal claims, such data will be stored only for the applicable statute of limitations, and only accessed in the event of a legal dispute. After this period, the data will be deleted, destroyed, or anonymized.

YOUR RIGHTS REGARDING YOUR PERSONAL DATA

As data subjects, you may exercise your rights under Article 11 of the Law by submitting your requests to us. Your application will be finalized free of charge within 30 days, unless it incurs an additional cost, in which case a fee may be charged as determined by the Personal Data Protection Authority. You have the right to:

• Learn whether your personal data is processed,
• Request information regarding such processing,
• Learn the purpose of processing and whether it is used accordingly,
• Learn the third parties to whom your data is transferred domestically or abroad,
• Request correction of incomplete or incorrect data, and notification of such correction to third parties,
• Request deletion or destruction of data when processing reasons cease, and notification of such action to third parties,
• Object to results arising against you from analysis by automated systems,
• Request compensation in case of damages due to unlawful processing.

CONSENT DECLARATION

I have been informed regarding the methods, purposes, and legal grounds for the processing and transfer of my personal data by Özel CM Park Ağız ve Diş Sağlığı Polikliniği, as well as my rights under the Law. I hereby declare that I have read and understood the Personal Data Protection and Processing Policy and the Retention and Destruction Policy, and that I am aware of my rights under Article 11 of the Law.

I give my explicit consent for my personal data collected verbally, in writing, or electronically, to be processed in a manner that is connected, limited, and proportionate to the stated purposes, and for receiving commercial electronic messages.
